Any chance it could be his anal glands? Those can get infected, and I've seen them lick themselves raw. That may not be the exact spot he's licking, but it's a pretty tough spot to hit, so maybe he's not built right to get the right spot and settles for something close.

The mite responsible for demodectic mange is actually present on most dogs. It usually only affects younger dogs who haven't developed a full immune system. A heart murmur itself is not a problem. Healthy hearts can have a murmur (which is what a "functional murmur" is), but in other cases, the murmur is a symptom of another problem.

I've heard of thyroid issues causing problems with a dog's ability to regulate heat. Vet can do a quick test. If that's the problem, meds are pretty cheap.
