From what I have been told 1/10 cc per 10lbs for Heart worms.

I HAVE BEEN USING IVOMIC FOR YEARS. 1/2 CC ON ALL DOGS 6 MONTHS &UP,BUT I USE A NEEDLE AND PUT IT UNDER THE SKIN NOT IN THE MUSCLE.NEVER HAD ANY DOGS DIE OR GET SICK.DO KNOW OF SOMEONE WHO OVER USED THIS METHOD."ONE SHOT A MONTH",AND KILLED HIS DOG.I USE IT EVERY 3 MONTHS,AND AN INTERCEPTOR PILL EVERY 2 MONTHS, AND HAVE NO PROBLEMS ANY WORMS OR KENNEL COUGH.

Zimectrin Gold has both Ivomec and Praziquantel. All worms with one wormer. Only issue is accurate dosing, but I have never had any issues.

Give this once a month and you will have no problems ever.
